How to Apply for the Service Unity Award
To apply for the Idaho HOSA Service Unity Award, each chapter must complete the official SUA Submission Form and report all eligible medical or health-related volunteer hours completed during the school year.

Record all qualifying medical/health-related volunteer activities your chapter participates in. Only hours that meet eligibility guidelines may be submitted. -

Idaho HOSA will total all verified hours and recognize the top three chapters at the State Leadership Conference with Gold, Silver, and Bronze certificates.
